• All students must have a physical every year.
• All incoming freshmen and anyone new to BK who did not have a physical last year will have to get one.
• If you are a new student to BK, but you had a physical last year, please submit a copy.
• Select "Athletic Forms" at the top of this page to download a copy of the "Health Examination and Consent Form". Select the icon in the middle of the page to upload your completed form.
• Students who do not have a physical will NOT BE ABLE TO RACE!!!!
