Registration Requirements
Please get your player’s physical and baseline concussion test if he hasn’t had them already for another sport this school year. Your son will not be allowed to try out for the team in February without a physical and test. The two forms that BK needs (Athletics Physical and Permission forms) are online at http://www.bkathletics.org/forms. Baseline concussion tests can be scheduled through St. Luke’s at https://www.stlukesonline.org/health-services/procedures/baseline-impact-testing or 381-4785. This test is $15 and required every 2 years. (PLEASE NOTE: The ONLY testing dates available before tryouts are Jan. 26 and Feb. 9.) Also required is a US Lacrosse membership that is valid through June 2017 – fee is $35/year. To sign up or renew your US Lacrosse number and to register with the Idaho High School Lacrosse League go to http://idaholacrosse.org/Registration. (Both can be done on this website.)

Winter Conditioning
Jack City Winter Conditioning will continue on Tuesdays & Thursdays from 3:30-4:30 and end on Thursday, Feb. 16. There will not be any winter conditioning on Tues., Feb. 21 & Thurs., Feb. 23.
Program Costs
Registration fees-$300 (Includes coaches’ stipends and league and referee fees)
Our program received help from the BK Parent’s Association ($2500) and the Annual Fund.
Spirit Pack - $148 for new players, $100 for returning players
USLAX membership online renewal - $35
Equipment – Varsity and JV players provide their own helmets (Varsity is required to have black/gold helmet), pads, gloves & stick. There is no rental equipment available at the high school level. If your player needs help with equipment, coaches may be able to help with some used/extra equipment.
